NEW PATIENTS: TIME TO BOOK

1. A new patient deposit is required prior to scheduling your appointment.

Please submit the $100 deposit when making your appointment request. Your appointment will be confirmed once the deposit has been received. Requests without a deposit will be removed, as they prevent other patients from accessing that time slot.

Please confirm you are in Washington State (WA).

2. Appointment confirmation will occur upon verification of the $100.00 deposit payment.

To pay, select "Home" at the top left of this page and scroll down to "Pay Now." Please indicate "FOC deposit" in the memo line. The deposit may be applied toward your visit cost or refunded if billing insurance. The deposit will be forfeited under the following circumstances:

A. Forms are not submitted at least 24 hours prior to your appointment.

B. Rescheduling occurs less than 48 business hours (Monday–Friday) before your visit.

C. You do not attend your scheduled visit.

If you join the waitlist, please provide your phone number so we can contact you.

3. Once your appointment is confirmed, you will receive an email invitation to join Charm.

A. To create a new Charm account, click the link in the email and follow the prompts.

B. If you already have a Charm account, please still use the email link, then log in with your existing credentials to link your account to this practice.

4. Upon entry into the Charm Portal, please complete all Questionnaires and Intake Forms.

All forms must be completed in full at least 24 hours prior to your appointment.
